Enrolling Your Preschooler
Enrollment into our Morning or Afternoon day program starts with submitting some basic information about your child to determine eligibility and medical necessity. Marty’s Orchid House is a day health program, requiring a prescription from your child’s Primary Care Provider and approval by insurance before your child starts. Our staff will walk you through this process!

Complete the Enrollment Application
Complete the online Enrollment Application (call our office if you need any assistance - (502) 290-2421).

A Nurse Will Call You
One of our dedicated nurses will contact you to discuss placement, eligibility requirements, and next steps. If your child has medical conditions, you may sign a release form for our team to obtain medical records from your child’s primary care provider.

Tour the Center
If your child is eligible for enrollment, you may schedule a tour of the facility. You can ask any additional questions you may have during the tour and obtain important information about our program.

A Care Plan Will Be Created
A Comprehensive Care Plan will be created for your child, signed by your child’s primary care provider and sent to insurance for approval. Once approved, your child is ready to start!
